We are scheduling in-office and telehealth visits, after you have made your appointment, our nursing staff will be contacting you prior to your visit. Have your medication list and medical information available so when they call, we can update your patient information. We ask that you send a copy of your insurance card and updated information to our office by fax @716-675-7101, mail, email, or by entering the patient portal on our website here. Our patient registration forms can be printed also and dropped off at the check in desk when you come for your appointment.
When you arrive for your appointment please call our office, we ask that you wait in your car or outside, please do not wait inside the building unless necessary. When an exam room is ready a nurse will call and meet you at the entrance to check your temperature.
We require that all patients and necessary assists wear a mask or face covering in our Office, if you forget a mask and we do not have any available, you will need to reschedule your appointment. If you choose not to wear masks during this time of uncertainty, we will ask that you cancel your in-office appointment and we can provide a telehealth appointment until masks are not a safety protocol.
